Short-term, goal-oriented support designed to help you regain your independence after a hospital stay, illness, or fall. We work alongside you to rebuild confidence and skills, stepping back as you are able to do more for yourself.
Our approach
Support that steps back as you step forward.
Reablement is different from ongoing care. It is a short-term, focused period of support with a clear aim, helping a person recover the skills and confidence to manage as independently as possible, in their own home.
Support is set around agreed goals and reviewed regularly. As someone regains ability, our involvement reduces, so the person is never supported more than they need.
What reablement involves
Goal-led support to rebuild independence
Every reablement plan is built around the individual and their goals. Support typically includes:
- Practical help with washing, dressing and mobility while strength returns
- Building confidence with everyday tasks such as cooking and moving around the home
- Working towards goals agreed with the person and their care team
- Regular review as independence increases, reducing support as it is no longer needed
- Close communication with GPs, discharge teams and family
- Encouragement and motivation throughout the recovery period
After reablement
What happens once goals are met
Many people need no further support once their reablement goals are reached. Where ongoing help is still needed, we can move straight into visiting or live-in care, with no disruption and no need to repeat the assessment from scratch.
